Responsibilities
Requirements
- Providing compassionate direct patient care in accordance with individualized care plans.
- Assist with medication administration and wound care.
- Monitor and document patient progress, reporting any changes to health care providers.
- Collaborate with healthcare professionals and family members to ensure optimal patient care.
- Active and valid state licensure (RN, LPN, or HHA certification as applicable) in Indiana.
- CPR Certification required.
- Minimum, 1 year experience in home health care or related field (preferred).
- Strong clinical skills and attention to detail.
- Reliable transportation and willingness to travel to patients' homes.
